Working within the Cambridge Health Alliance as a primary care team nurse in a Patient Centered Medical Home in the Family Practice setting.
Managing patients with chronic illnesses such as Diabetes, COPD,CHF, HTN, Asthma, Depression etc.
Managing/monitoring patient reports to ensure quality metrics are met annually including: Diabetics at A1C goal <8.0, Hypertensive patients at goal of BP <140/90, Pediatric Asthmatic patients receiving annual flu vaccines and Asthma Action Plans determined annually etc.
Experience with telephone triage, walk-in triage. Case management, team nursing models. Experience with electronic medical records.

Special interest in Asthma education as well as Substance Use Disorder-recently took part in a grant funded project focused on introducing Buprenorphine management in the Primary Care setting.
I am 1 of 7 nurses in my family and grew up surrounded by medicine. I manage several chronic illnesses of my own as well, which gives me a unique insight to the patient side of things usually. I feel it's a strength in my ability to provide patient care, I can truly understand what they are going thru in most cases.
